Electronic Palliative Care Coordination Service

What is it?               

It is a way to provide up to date information on patients’ end of life preferences. It is a way to help patients discuss their preferences about where they die and the transition of information relating to their care. As part of this work the General Practice Repository of Clinical Care (GPRCC) and the Medical Interoperability Gateway will soon be connected so that everyone involved in your care knows your wishes.

 

Who uses it?

Healthcare professionals can access this up to date information and follow the patient’s requests. These healthcare professionals can edit the preferences if the patient asks them to.

 

What are the benefits?

It can help to minimise the same discussion being had with multiple care providers. It is also a valuable resource in ensuring that all care providers have the patient’s most up to date preferences. This reduces stress and saves clinical time across the Health Community.
